Insurance Information

Dr. Sidhom participates in a wide range of medical plans and managed care organizations. However it is important to check your insurance providers list of participating physicians to be sure your program allows you to be seen at our office.

Initial Visit

On your initial visit, please bring:

• Your insurance card
• Personal identification
• List of allergies
• List of current medications (prescription or over-the-counter) including your dosage and frequency
• Your referral, if required by your insurance provider
• Medical records from your referring physician
